I wanted to screenshot it, but am afraid to replicate it and have to redo the field manually. I was able to get it to load on Edge, where it always loads, however on Edge, when I tried to link the "make" fields to the PDF, it would create a blue fillable field the size of a tiny dot in the top left corner of the PDF - I was able to resize it, but it would disappear whenever I tried moving it. I had a problem using Chrome (the browser I always use) because the PDF form usually displayed on the right (of the split screen with form on the left) just kept saying "Loading PDF" and never loaded it up - I am having this problem the whole time when using your site on Chrome browser.
